REQUIRED PLAYER FORMS

Must Be Completed by All Players and Kept in Coaches Binder

Here are the required forms that each player must complete and return to the coach PRIOR to participating in any CYO games, and ideally before practices begin.  CYO administration will email these forms with instructions for families to complete all three, print them, and bring them to your team's first practice/parent meeting.  It is the coaches' responsibility to ensure that all three forms are collected for each player on the team.
